Licensed Practical Nurse - OB Intake
Community Health Services
Yakima Medical-Dental Clinic
Full-time, 40 hrs/wk

The LPN - OB Intake will provide pregnant women with integrated clinic and program services as part of our Community Health Services team. Spanish language fluency is required in this role.

  • The ability to assess and determine the needs of clinic patients.

What you'll do:

  • Provide OB intakes for clinic by completing prenatal history and ordering appropriate labs.
  • Work with care team including physicians, nurses, social workers, case managers and dieticians to assess risk level and develop care plan with the goal of promoting healthy birth outcomes.
  • Work with clients to schedule home or office visits to carry out health coaching and interventions, and encouraging clients' active participation in their care plan.
  • Provide health education for prenatal, postpartum and newborn care.
  • Partner with client's OB care team to support healthy birth outcomes.

Minimum Qualifications:

  • High school diploma or GED.
  • Graduation from an accredited LPN program.
  • LPN license for the state of practice, CPR certification within 3 months of employment.
  • Bilingual in English/Spanish with demonstrated ability to communicate in Spanish at level 10 on the ALTA language proficiency scale.
